Diaspora Primary Care is designed to make primary care more accessible, personal, and connected. Your monthly membership gives you ongoing access to your care team without having to wait until something is wrong to reach out.
Whether you’re joining on your own, with a partner, or as a family, you’ll receive the same thoughtful, affirming care.
Option I- Individual
$80 per month | One adult
Includes:
• Primary and preventive care
• Annual wellness visits
• Routine follow-up appointments
• Same-day or next-business-day appointments when available
• Telehealth visits throughout California
• Chronic condition management
• Medication management and refills
• Routine lab ordering and review
• Sexual and reproductive healthcare
• Gender-affirming primary care
• Preventive screenings and health planning
• Care coordination and specialist referrals
• Secure messaging with your care team
• Psychiatric consultation and medication management when clinically appropriate*
• Access to integrated behavioral health resources
Option II- Individual and Spouse
$120 per month | Two adults
Includes everything in the Individual Membership for two adults, including:
• Primary and preventive care for both members
• Individual wellness visits
• Routine and acute-care appointments
• Medication management
• Lab ordering and review
• Sexual and reproductive healthcare
• Gender-affirming care
• Chronic condition management
• Psychiatry and behavioral health integration*
• Secure care-team messaging
• Care coordination and specialist referrals
Membership isn’t limited to legally married couples. Partners and spouses are welcome.
Option III- Individual and Family
$80 per month +40 for each additional member | Household membership
Includes everything in our Individual Membership for eligible members of your household, plus:
• Primary care for the family
• Preventive and wellness visits
• Adolescent healthcare for ages 12+
• Routine and acute-care appointments
• Medication management
• Lab ordering and review
• Sexual and reproductive healthcare
• Gender-affirming care
• Mental health and psychiatric support*
• Care coordination across family members when appropriate and authorized

$60 per month
- Direct text and email access to the Diaspora healthcare team
