Healthcare That Meets You Where You Are.
At Diaspora Primary Care, we believe healthcare should be accessible, affirming, and built around relationships. Our mission is to provide comprehensive primary care that supports the whole person by integrating physical, mental, and preventive healthcare into one seamless experience. Through longer conversations, collaborative care, and a commitment to equity and inclusion, we empower adolescents (12+) and adults across California to take an active role in their health with care that is convenient, connected, and centered on what matters most to them.
What We Offer:
Primary & Preventive Care
Routine primary care for adolescents and adults, including annual wellness visits, preventive screenings, chronic condition management, health education, and everyday medical concerns. Care is personalized, culturally responsive, and focused on your health beyond a single appointment.
Mental Health & Psychiatry
Integrated psychiatric care, including mental health evaluations, diagnosis, medication management, and ongoing follow-up. Our medical and mental health providers can collaborate to consider both physical and emotional health when developing your treatment plan.
Gender-Affirming Care
Affirming medical care for transgender, nonbinary, and gender-diverse patients, including hormone therapy, ongoing medication management, routine health monitoring, preventive care, and coordination with other affirming providers when needed.
Sexual & Reproductive Health
Confidential, judgment-free care for sexual and reproductive health, including STI testing and treatment, contraception, PrEP/PEP, sexual wellness concerns, reproductive health education, and routine screenings.
Women’s Health
Whole-person care across different stages of life, including menstrual concerns, contraception, reproductive health, pregnancy and postpartum support, perimenopause and menopause, preventive screenings, and other common health concerns.
Chronic Care & Medication Management
Ongoing support for conditions such as high blood pressure, diabetes, high cholesterol, thyroid concerns, asthma, and other chronic health needs. We can also manage and monitor medications, coordinate labs, and help patients make sense of care that may involve multiple specialists.
What You Can Expect
A more thoughtful approach to everyday healthcare.
Whole-Person Care
- Culturally Responsive Care
- Care that approaches culture, identity, and lived experience with curiosity and respect rather than assumptions.
- LGBTQ+ & Gender-Affirming
- Inclusive, affirming healthcare where you shouldn’t have to educate your healthcare provider before receiving care.
- More Time, Less Rush
- A care model designed to create more room for questions, conversation, prevention, and ongoing relationships with your healthcare team.

Your physical health and mental health don’t live in separate bodies. Neither should your healthcare.
When appropriate and with your consent, our primary care and psychiatric providers can collaborate on treatment, medications, labs, symptoms, and overall health goals. That means fewer disconnected pieces of care and a healthcare team that can see the bigger picture.
“We created Diaspora Primary Care to bring physical and mental healthcare together in a way that feels thoughtful, affirming, and human.
Our goal is simple: Care for the whole person, not just the reason for the appointment.”
