Healthcare shouldn’t feel like another full-time job.
Diaspora Primary Care is a membership-based primary care practice designed to make getting care simpler, more personal, and easier to navigate. Instead of waiting until something is wrong or trying to squeeze everything into one rushed appointment, your membership gives you an ongoing relationship with a care team that gets to know you over time.
We provide primary care, preventive care, mental health and psychiatric services, sexual and reproductive healthcare, gender-affirming care, medication management, and chronic condition support through one connected model.
More access. More continuity. Less healthcare ping-pong.
Direct Primary Care, or DPC, is a healthcare model built around a direct relationship between you and your primary care practice.
Instead of billing insurance for each primary care visit, members pay a predictable monthly membership fee for access to the care included in their membership.
That allows us to spend less time designing care around billing requirements and more time focusing on the person receiving it.
Your membership is not health insurance, and it does not replace insurance for services outside of Diaspora Primary Care. It is a different way of accessing your everyday healthcare.
How It Works
1. Choose Your Membership
Choose the membership that best fits you and your household. We offer memberships for individuals, couples, and families, with straightforward monthly pricing and clearly defined benefits. No decoding required.
2. Get Established With Your Care Team
New members complete an intake so we can understand more than what fits into the “reason for visit” box. We’ll review your medical history, medications, current concerns, preventive care needs, mental health history when relevant, and the healthcare priorities that matter to you. This is also an opportunity to talk about previous experiences with healthcare, including experiences that may have made it harder to feel heard, respected, or comfortable seeking care.
3. Build Your Care Plan
Healthcare is rarely one-size-fits-all. Together, we’ll identify what needs attention now, what should be monitored over time, and what preventive care makes sense based on your individual health history, body, age, identity, family history, and goals. Your care may include preventive screenings, chronic condition management, medication management, sexual or reproductive healthcare, gender-affirming care, mental health support, psychiatric care, or coordination with outside specialists.
4. Reach Out When You Need Care
Primary care shouldn’t disappear between annual physicals. Membership gives you ongoing access to your care team for the healthcare needs that come up throughout the year. Depending on the concern, that may mean scheduling an appointment, checking in about symptoms, reviewing medications, discussing test results, or deciding together whether you need a higher level of care.
5. We Keep Looking at the Whole Picture
Mental health affects physical health. Hormones can affect mood. Sleep can affect nearly everything. Medications prescribed by one provider can affect treatment being managed by another.

Your body did not organize itself into medical specialties, so we try not to treat it that way. Our integrated model allows us to consider the connections between your physical health, mental health, medications, relationships, environment, identity, and lived experience while still practicing evidence-based medicine.

What Isn’t Included?
Your Diaspora Primary Care membership is not health insurance. Services provided outside of our practice may have separate costs. These can include: laboratory testing, imaging, medications, specialist visits, emergency room care, urgent care outside our practice, hospitalization, procedures performed by outside clinicians, and other services that require an outside facility or provider. For this reason, many DPC patients choose to maintain health insurance for unexpected, emergency, specialty, and hospital care. When outside care is needed, our role doesn’t simply end at the referral. We can help you understand why something is being recommended and coordinate with other members of your healthcare team when appropriate.
Can I Still Have Health Insurance?
Absolutely. Direct Primary Care and health insurance serve different purposes. Think of your Diaspora membership as the home base for your everyday healthcare. Insurance can still be used for covered services outside of our practice, including specialists, hospital care, imaging, emergency services, and other healthcare expenses. Having insurance does not prevent you from becoming a Diaspora Primary Care member.
What If I Already Have a Primary Care Doctor?
That’s okay, too. Some people come to Diaspora because they are looking for a new primary care home. Others may be transitioning from another physician or healthcare system.
If you decide to establish primary care with us, we can review your existing records and help create continuity between your previous care and your new care plan. You do not have to arrive with your entire medical history perfectly organized.
What Happens If I Need a Specialist?
Sometimes good primary care means knowing when something needs more specialized attention. When appropriate, we can recommend or refer you for specialty evaluation and help coordinate that care. Depending on your insurance plan, network requirements, and the specialist, outside services may be billed separately through your insurance or directly by that provider.
What About Emergencies?
Diaspora Primary Care is not an emergency medical service. If you are experiencing a medical or psychiatric emergency, call 911 or go to the nearest emergency department. Your membership should make everyday healthcare easier to access, but it does not replace emergency services, hospitalization, or other higher levels of care.
Telehealth + In-Person Care
Diaspora Primary Care is designed to make healthcare accessible even when getting to a doctor’s office isn’t. Many appointments can be completed through secure telehealth, including follow-up care, medication management, mental health and psychiatric care, review of laboratory results, sexual health consultations, chronic condition follow-up, and many routine primary care concerns. When an in-person examination, testing, imaging, procedure, or other hands-on care is medically necessary, we’ll help determine the appropriate next step.
Healthcare That Leaves Room for Context
A lab value matters…but so does the person attached to it. We recognize that health is shaped by more than biology. Culture, race, gender, sexuality, family, finances, work, trauma, discrimination, relationships, community, and access to resources can all influence how people experience their health and the healthcare system. Our approach is culturally responsive, LGBTQ+ and gender affirming, trauma informed, and grounded in clinical evidence. You don’t need to belong to any particular community to receive care here.
Ready for Healthcare That Feels More Connected?
Whether you’re managing an ongoing condition, trying to stay ahead of your health, looking for affirming care, or simply tired of feeling rushed through appointments, Diaspora Primary Care offers another way to experience primary care.
Still Have Questions?
That’s completely reasonable. Direct Primary Care is still unfamiliar to many people. Visit our FAQ to learn more about membership, insurance, prescriptions, labs, telehealth, psychiatric care, family memberships, cancellations, and what to expect as a Diaspora Primary Care member.
